How to Find a Medical, Dental or Vision Provider

Login to www.myuhc.com to view providers and facilities in your area.
If you are not currently a member, you can visit www.uhc.com and search for a provider in the UHC Choice/Choice Plus Network (Choice/Choice Plus Harvard Pilgrim in Massachusetts) network.
Dental Provider
To search for a dentist on www.cigna.com, visit the site and click “Find a Doctor, Dentist or Facility.” Follow the prompts on screen and when asked to choose your plan, select:
- DMO Plan: CIGNA DENTAL CARE DHMO > Cigna Dental Care Access Plus
- Essential or Enhanced Plan: DPPO/EPO > Total Cigna DPPO
